Makeup Consent

    Glam In The City Spa Bus requires you, the parent, to sign this waiver. As part of the spa experience, age appropriate makeup and spa treatment products will be applied. This includes, but is not limited to lip gloss and eyeshadow. Makeup products are hypoallergenic and makeup applicators are new and disposable. I acknowledge that any of the beauty or spa treatments are for entertainment purposes only and Glam In The City Spa Bus employees are not licensed cosmetology professionals. Glam In The City Spa Bus employees will not cut, file, trim, push back cuticles or do any services provided at a certified nail salon. Glam In The City Spa Bus is an event planning company and the spa party themes are for entertainment only.

 

Photo Release For Minors

    I give Glam In The City Spa Bus permission to use my child’s likeness, image, voice, and/or appearances as such that they may be embodied in any photos, video recordings, digital images, and the like, taken or made on behalf of Glam In The City Spa Bus party activities. I agree that Glam In The City has complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Glam In The City Spa Bus parties. These uses include, but are not limited to reprints, publications, advertisements, social media, and any promotional materials in any media now known or later developed, including the internet. I acknowledge that I will not receive any compensation, etc., for the use of such pictures, etc., and hereby release Glam In The City Spa Bus from any and all claims which are in any way connected with such use. I have read and understood this content and release.

 

Food Allergies

    Your child will be participating in entertaining spa-like activities. Some of these activities involve food products as part of the experience. We know that many children have allergies and some of these allergies can be life threatening. We take many precautions to ensure that all of the children are safe at all times. Glam In The City Spa Bus will not be responsible for any allergic reaction or life-threatening incident that may occur. If your child has any KNOWN food allergies or intolerances that will prevent her from participating in these activities, we will provide an alternate option at your request.

Coronavirus/COVID-19

    I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Glam In City Spa Bus has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Glam In The City Spa Bus can not guarantee that I will not become infected with the Coronavirus/COVID-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, party staff, and other guests and their families. I voluntarily seek services provided by Glam In The City Spa Bus and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I and my family must comply with all set procedures to reduce the spread while attending any Glam In The City Spa Bus event.

 

Liability

    We strictly provide entertainment. We can not be held responsible for accidental damage caused by your child. Glam In The City Spa Bus assumes no responsibility or liability for accidents or damage caused during the party. In the event of damage or loss of our inventory and/or damage or loss to the party bus, the responsibility lies entirely on our client and/or guardian. Glam In The City Spa Bus is not responsible for allergic reactions and/or any medical injuries from the food, beverages, wearing costumes, or any activity taking place before, during, or after the party. I hereby release and agree to hold Glam In The City Spa Bus harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by an act, or that otherwise arise in any way in connection with any services received from Glam In The City Spa. I understand that this release discharges Glam In The City Spa Bus from any liability or claim that I, my family, or any personal representatives may have against the business with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to any services received from Glam In The City Spa Bus. This liability waiver and release extends to Glam In The City Spa Bus together with all owners, partners, and employees.
